Disabled Permit Applications

Dear Source:
It would be interesting to know just when the application form for these permits will be available on the Internet.
I tried to renew my permit but was told I’d need to:
1) Get a new application (for $2),
2) take it to a doctor for verification (a costly office call)
3) return the completed form for ‘processing.’
I don’t mind paying the fees for this government but in the past all I had to do was hand in my old permit plus the fee for the new one and wait a few minutes.
My recent visit to the ‘new facility’ caused me a half hour wait to be told what to do and then I’d have to wait ‘by a window’ to purchase the new permit.
Your new "simple" procedure takes more time than it should.
You need to think and put in place the entire procedure before messing up the old one!
