Innovative Telephone Lifeline Recertification Moving to Business Offices

Effective Nov. 1, customers whose Lifeline certification will expire on Nov. 30 should visit the Innovative Business Offices for recertification. Offices that can process applications are located at Tutu Park on St. Thomas and Estate Diamond on St. Croix.

Customers must come in between Nov. 1 and Nov. 20 for recertification. For those customers that have certifications expiring in future months, they will receive a reminder letter from Innovative to renew their Lifeline benefits.

Customers must bring the following requisite documents with them when they apply for recertification:

• Acceptable and valid forms of identification that confirm legal name, date of birth and social security number

• Proof of program eligibility or proof of income eligibility

The Department of Human Services will no longer process Lifeline applications after Nov. 1.
